School Closure Notifications
Please reach out to your School Participation Division with any questions regarding the completion and/or submission of this form.
When reporting a closure, please have the following information:
- Closure Date (the date the school ceased or anticipates to cease providing educational instruction in all programs of study, and if available;
- Arrangements made by and for students to complete their education;
- Arrangements made for maintaining the academic records; and
- Information about whether the school was covered by a state tuition recovery fund or performance bond.
