School Closure

The School Closure Form allows partners to timely notify the Department of its anticipated and/or actual closure date, and to provide required information for processing the closure.

School Closure Notifications

Please reach out to your School Participation Division with any questions regarding the completion and/or submission of this form.

When reporting a closure, please have the following information:

  • Closure Date (the date the school ceased or anticipates to cease providing educational instruction in all programs of study, and if available;
  • Arrangements made by and for students to complete their education;
  • Arrangements made for maintaining the academic records; and
  • Information about whether the school was covered by a state tuition recovery fund or performance bond.
