Ford Federal Direct Loan Program: Economic Hardship Deferment Request, Public Service Deferment Request, Education Related Deferment Request, Parental Leave/Working Mother Deferment Request, Temporary/Total Disability Deferment Request, Unemployment Deferment Request and In-School Deferment Request. When providing these forms to students, please pay special attention to the eligibility criteria specified on each form. The 1996-97 In-School Deferment Request form may be ordered in bulk from the Direct Loan Program, Loan Origination Center, School Relations Staff. An order form is enclosed for your convenience. Schools may photocopy the other forms or download an electronic version of the forms from the Student Financial Assistance Bulletin Board System. They cannot be ordered in bulk. However, if you should ever need an additional copy, please contact the School Relations Staff and request a copy of this bulletin.
