Dear Colleague:

Enclosed are seven 1997-98 deferment forms for the William D.
Ford Federal Direct Loan Program:

- Economic Hardship Deferment Request
- Public Service Deferment Request
- Education Related Deferment Request
- Parental Leave/Working Mother Deferment Request
- Temporary/Total Disability Deferment Request
- Unemployment Deferment Request
- In-School Deferment Request

Also enclosed are three 1997-98 forbearance forms for the William
D. Ford Federal Direct Loan Program:

- General Forbearance Request
- Loan Debt Burden Forbearance Request
- Internship Residency Forbearance Request

When providing these forms to students, please pay special attention
to the eligibility criteria specified on each form.

The 1997-98 In-School Deferment Request form and the 1997-98
General Forbearance Request form may be ordered in bulk from the
Loan Origination Center. An order form is enclosed for your
convenience. The other forms cannot be ordered in bulk. Schools
may photocopy them or download an electronic version of the forms
from the Student Financial Assistance Bulletin Board System. 32If
you need an additional copy of this bulletin, DLB #29, please contact
the Loan Origination Center.

If you have questions regarding the use of these forms, please
contact the Direct Loan's Relations Services Automated Call Routing
System at 1-888/445-7745. Please select menu number three for
instructions for your specific Loan Servicing Center.
