The Fiscal Operations Report for 2013-2014 and Application to Participate for 2015-2016 (FISAP) has been approved by the Office of Management and Budget (OMB). We have posted the FISAP form, instructions, and Desk Reference to our Information for Financial Aid Professionals (IFAP) Web site. The FISAP itself will be available on the eCampus Based (eCB) Web site by August 1, 2014.

We have also posted the Technical Reference for the 2015-2016 FISAP that provides technical specifications and record layouts for schools or other organizations that wish to design custom software systems.

These documents will assist schools in completing the 2015-2016 FISAP.

Reminder: As announced in the Notice of the 2014-2015 Award Year Deadline Dates for the Campus-Based Programs published in the Federal Register on January 15, 2014 (79 FR 2646), the date by which a school must submit its 2015-2016 FISAP and the required signature pages is Wednesday, October 1, 2014.

In addition to the posting of these updated documents, there are a number of important changes to Campus-Based processing. We cover this information in the following order:

Electronic Signature for eCampus-Based Processes

Beginning August 1, 2014, schools will have the option to electronically submit the signature pages for the FISAP and all other forms within the eCB System that require the school’s Chief Executive Officer’s (CEO) signature. Electronic Signature (eSignature) eliminates the need for schools to send hard copy signature documents printed from the eCB System to the Department of Education. Schools will continue to have the option to send signature documents manually (via paper).

eSignature – School submits signature pages electronically. The CEO signs the FISAP electronically through the eSignature tab within the eCB Web site at https://cbfisap.ed.gov. The eSignature instructions are available through a link on the eCB Login page and through the Help tab within the eCB Web site.

Notes:

  • The FISAP form must be electronically submitted before the FISAP eSignature may be submitted.

  • The signature of the CEO, as designated on the school’s Program Participation Agreement (PPA), remains the required signature for all eCB forms.

  • Before signature pages can be submitted electronically using eSignature by the school’s CEO, he or she must initiate several actions. More information about these actions, eCB user registration, Two Factor Authentication (TFA), and eSignature is provided in a June 26, 2014 electronic announcement.

If a school chooses to continue submitting paper signature pages, it is required to send the hard copy of the combined certification and signature pages—containing Section A information and containing, in Section B, the original signature of the CEO—to the address below. Print out these pages by clicking the Self-Service tab, going to “Forms and Reports,” selecting “Signature Pages,” and then selecting “File” and “Print.”

Note: This is a new address. Be sure to update your school’s records accordingly.

Regardless of whether a school uses the new eSignature process or continues submitting paper signature pages, the signature of the CEO, as designated on a school’s Program Participation Agreement (PPA), remains the required signature for all eCB forms. In addition, signature page documents must be submitted by midnight (ET) of the deadline date. A school should account for the additional time for delivery when submitting paper documents.

Lobbying Form Now Required as an Electronic Submission

The Lobbying Form (SF-LLL) must be submitted electronically via the eCB Web site. Please see details on Page 15 of the Desk Reference to determine if your school is required to complete an SF-LLL form. The SF-LLL form has been added to the Setup tab within the eCB Web site. As with other eCB forms, a school can use eSignature or mail paper signature pages to complete the submission.

Changes to the FISAP

No changes have been made to the FISAP data entry fields. The award year and date references have been updated.

Changes to FISAP Instructions

In addition to annual award year and date references, the following changes have been made to the FISAP Instructions:

Introduction

  • Updated deadline dates for the 2014-2015 Award Year.
  • Clarified information concerning reporting for more than one institution.
  • Added clarification of the deadline date for the FISAP signature page.
Part I, Section A.
  • Clarified instructions for Additional Institutions.

Part I, Section B.

  • Added FISAP signature options to include FISAP eSignature option, electronic submission of the Lobbying form, and Lobbying Form eSignature option.
  • Added clarification of the deadline date for the FISAP signature page.

Part II, Section F.

  • Added information concerning the use of EDExpress Packaging Module reports to assist in completing the Part II and Part IV income grids.

Part III, Section A.

  • Field 18 – Added a note providing the URL for the list of Tribal Colleges and Universities.

Part III, Section C.

  • Field 2.1 – Updated the new Servicer address.
  • Field 2.2 – Updated the Electronic Announcement reference and the e-mail address for Total and Permanent Disability (TPD) discharge information.

Part IV, Section A.

  • Field 1 – Added note to clarify the Federal Supplemental Educational Opportunity Grant (FSEOG) underuse penalty.

Part V, Section A.

  • Field 1 – Added note to clarify the Federal Work-Study (FWS) underuse penalty.

Part V, Section G. (FWS) underuse penalty.

Part V, Section G. larified the calculation concerning the minimum amount of FWS federal funds a school must spend on community service jobs by restating the third bullet point.

Part VI, Section A.

  • General Instructions for Section A – Updated details for using the EDExpress Packaging Module reports and queries to assist with the FISAP Part VI income grid.

Other Reminders - Check Access to FISAP on the Web

If it has been more than 90 days since an eCB user has logged on to the eCB Web site, the user’s password will need to be reset. Information on how to reset a password or register as a Destination Point Administrator, and contact information for assistance is provided in the Desk Reference. The Desk Reference is also available online at the login page for the eCB Web site. All users should ensure that their access to the eCB Web site application is functional before needing to enter data to file the school’s FISAP.
