Posted on 08-31-2007 As the U.S. Citizenship and Immigration Services (USCIS) gradually reduces the number of field offices that process the Immigration Status/Document Verification Request, G-845 form, they are notifying Federal Student Aid of the changes. We first informed the community about USCIS field office closures in an Electronic Announcement posted on April 11, 2007. New Field Office for Schools in Utah, Minnesota, North Dakota, and South Dakota Effective August 20, 2007, the Salt Lake City, UT and St. Paul, MN Immigration Status Verification (ISV) Units no longer exist. Manual G-845 cases are now being handled by the Dallas, TX field office. Schools in Utah, Minnesota, North Dakota, and South Dakota that previously sent G-845 forms to Salt Lake City and St. Paul will now send the forms to: US Citizenship and Immigration Services New Field Office for Schools in Ohio In addition, effective September 1, 2007, the Cleveland, OH Immigration Status Verification (ISV) Unit will no longer exist. Manual G-845 cases will be handled by the Chicago, IL field office. Schools in Ohio that previously sent G-845 forms to Cleveland will now send the forms to: US Citizenship and Immigration Services These are the only field offices that have changed at this writing. We will continue to update you as additional changes occur. Updated USCIS Field Office List The office to which a school will submit the new G-845 form will depend on the state or territory, and sometimes the county, in which the school is located. The USCIS has provided Federal Student Aid with a chart listing the field offices that will oversee each state, territory, and/or county, and the addresses for those offices. We have updated the USCIS field office list and it is provided as an attachment to this announcement on the Information for Financial Aid Professionals (IFAP) Web site. Use Attachment A to determine the field office and address to which you will send the new G-845 form, based on the location of your school. Note: The zip code for the Chicago field office was incorrect in the attachment to the Electronic Announcement posted on May 22, 2007. Attachment A to this announcement now contains the correct zip code for Chicago, which is "60605." Please ensure that your records are updated. We apologize for the inconvenience this may have caused. New G-845 Form We are also attaching a copy of the new G-845 form as Attachment B to this announcement on the IFAP Web site .
