| PublicationDate: 3/3/99 Summary: Title IV WAN Enrollment Document Author: TIV/WAN - Title IV Wan Customer Service Posted on March 3, 1999 TO: All Destination Points FROM: Title IV WAN DATE: March 2, 1999 RE: Title IV WAN Enrollment Document The Department of Education has approved a new enrollment document, "Enrollment to Participate in Title IV WAN" which is to be used for all new enrollment and changes to enrollment for Title IV WAN. It takes the place of the enclosures in Action Letter #4 November 1996 (GEN-96-20): Customer Initiation Document, Letter of Application, Participation Agreement. If you are presently enrolled in Title IV WAN, you are not required to submit this document, unless you wish to: 1) Change or add information and/or services for your Destination Point or 2) Enroll an additional Destination Point (receive a new TG#, or mailbox). As a current user the title, Destination Point Administrator (DPA), takes the place of your current title as Destination Point Contact, and has new responsibilities. The DPA is now required to certify that every user of the Destination Point is authorized to use it by filling out a "Title IV WAN User Statement" (Attachment B) for each User of the Destination Point. This document should be kept on file at each Destination Point. Do not send this document to Title IV WAN.
