Effective November 17, 1997 you will be able to order the Renewal Application Instruction booklet by following the procedure outlined below: 1.) Dial 800-615-1189 2.) Press option 1 if you have a touch tone phone or stay on the line if you have a rotary phone. 3.) Type in the last five digits of your Title IV WAN id. 4.) Press 9 for the Renewal Application Instruction Booklet request. 5.) Press 1 to confirm. 6.) Enter quantity you want to receive followed by a # sign. (Example. 100# for 100 booklets) 7.) Press 1 to verify how many booklets you want and that completes the request or press 2 to return to step #6 above.
