As we indicated in the Direct Loan Bulletin (DLB 99-14) and (DLB 99-19), we now have an "original" and a "revised" version of the Master Promissory Note (MPN) for Direct Loans. Attached to this announcement are the pdf files for the "revised" MPN, the Stafford Borrowers Right’s and Responsibilities statement, and the Disclosure Statement that will be used for the MPN.

“Revised” Version of the MPN:

There are two versions of the front page of the "revised" MPN form (one version has data labels and the second version is without data labels). Both versions are provided in pdf files below. Data labels refer to all the section boxes and labels that tell a person what data or information is shown in that box. An example of a data label is: Name (last, First, M.I.). The notes without data labels have large white spaces where your printer will add the lines and data labels as well as the actual data when it prints.

The following pdf file contains the second page (Disclosure Of Loan Terms) and the third page (Instructions and Notices) for the "revised" MPN. These two pages go with both versions (with data labels and without data labels) of the front page of the "revised" MPN form that were provided in pdf format above.

When you print these pdf files, print these with the "shrink to fit" option to ensure that you get the entire text. (These forms are intended for your viewing and information only. Actual forms that the school intends to use should be ordered from the LOC.)

"Original" Version of the MPN:

You have previously been provided with the pdf files for the "original" MPN. If you wish to view or print the "original" MPN, that version can be accessed (as an attachment) at the end of the document GEN-99-8.


Stafford Borrowers Right’s and Responsibilities Statement:

We are attaching pdf file (54510902.pdf) for the Stafford Borrowers Right’s and Responsibilities statement that will be used with the Master Promissory Note. When you print this pdf file, print this with the "shrink to fit" option to ensure that you print the entire text.

MPN Disclosure Statement:

The Disclosure Statement has been redesigned so the school can print the following pdf file (eds.pdf) out and use this printed form to provide the student with a Disclosure Statement. To print this form properly; first go to Print Setup, then to Properties, then download the Fonts as bitmap.
