Subject: Corrections to GEN-11-19/FP-11-01 (Revised Loan Discharge Application: Unpaid Refund)
Summary: This letter provides corrections to GEN-11-19/FP-11-01, which announced the approval of the revised version of the form called “Loan Discharge Application: Unpaid Refund” for use by borrowers in the William D. Ford Federal Direct Loan (Direct Loan) Program and in the Federal Family Education Loan (FFEL) Program.
Dear Colleague:
Attached to this letter (in both Microsoft Word and PDF format) are corrected versions to the Loan Discharge Application: Unpaid Refund form that was announced in GEN-11-19/FP-11-01. The copies of the form that were attached to GEN-11-19/FP-11-01 contained minor, typographical errors related to the numbering of the items in Section 3 of the form. Though not substantive in nature, the errors could cause confusion for some applicants.
The Loan Discharge Application: Unpaid Refund form is used by Direct Loan and FFEL Program borrowers to apply for a loan discharge when a portion of a loan was not refunded by an institution in accordance with the regulations governing the Title IV federal student aid programs at 34 CFR 668.22. The revised Loan Discharge Application: Unpaid Refund is approved under OMB Control Number 1845-0058 and included only minor changes from the previous version that is currently in use.
Implementation of the Revised Form
Program participants should begin distributing the attached OMB-approved revised form immediately. Beginning June 1, 2012, only the attached revised form may be provided to borrowers. However, the previous versions of the form may continue to be processed after that date.
All other conditions on the use of this form, as specified in GEN-11-19/FP-11-01 continue to apply.
