Subject: Minor Correction to Total and Permanent Disability Discharge Application
Summary: The attachment to this letter provides a new PDF version of the recently posted Discharge Application: Total and Permanent Disability.
Dear Colleague:
Attached to a Dear Colleague Letter posted on February 9, 2009 (DCL GEN-09-01, FP-09-01, and CB-09-01) were both PDF and Microsoft Word versions of the OMB approved Discharge Application: Total and Permanent Disability for use by borrowers in the Federal Family Education Loan (FFEL), William D. Ford Federal Direct Loan (Direct Loan), and Federal Perkins Loan (Perkins Loan) programs, and by recipients of grants under the Teacher Education for College and Higher Education (TEACH) Grant Program. We have discovered the inadvertent omission of the last several words of the last sentence in Section 6 on page 4 on the PDF version only - the Microsoft Word version is correct.
Please use the PDF version from this letter instead of the version attached to the earlier letter.
