Subject: G-845 to be sent to Los Angeles DHS-USCIS Field Office

In an Electronic Announcement dated June 29, 2015, we notified the community of the revised Document Verification Request G-845 Form (OMB No. 1615-0101, expires 05/31/2018).

We have been notified by the Department of Homeland Security, U.S. Citizenship and Immigration Services (DHS-USCIS) that the submission of the G-845 Form for students required to undergo additional verification of their eligible non-citizenship status must be submitted to the Los Angeles field office, regardless of the geographic location of the institution submitting the G-845 Form. Effective immediately, institutions must mail these forms and attachments to:

The regulations at 34 CFR 668.135 outline the process an institution must take when a noncitizen student is required to undergo additional verification of their eligible non-citizenship status using the G-845 process. That process requires an institution to complete parts 1 and 2 of the G-845 Form, attach copies of the front and back sides of immigration-status documents received from the student, and submit the form and attachments to the DHS-USCIS Los Angeles field office. Exception for Battered Immigrant-Qualified Aliens with a Status Under VAWA

Note that if a student with status under the Violence Against Women Act (VAWA) requires third-step verification as a battered immigrant-qualified alien, the G-845 Form and attachments must continue to be sent to the Buffalo field office. Please send VAWA related G-845 Forms and attachments to:
